Crank Extraction - Threading Prob...

I can get my left crank off easily enough, but the other side is not playing. The tool is threading in, but when you start the extraction it just pulls out after a while.
a) is it worth trying a different tool (I have the one which does NOT have a handle built in)?
b) if the thread's gone, what are the options for removing the crank??
